Bridge to Next Steps Program is a program that helps patients and families as they transition from active treatment into post-therapy care. The program consists of two specialized visits to provide education, support and a treatment summary called a passport, which is a wallet size summary of care with long-term follow up recommendations:
 

• Visit 1 (prior to end of treatment): Survivorship Provider
• Visit 2 (shortly after end of treatment): Survivorship Provider and Psychologist
   

Comprehensive Next Steps Survivorship Clinic is a long-term follow-up program made up of expert specialists across Children’s Wisconsin that cares for cancer and BMT survivors who:

• Received care at Children’s Wisconsin or elsewhere
• Were diagnosed before age 30, regardless of current age
• Completed treatment a minimum of two years ago
• Received a bone marrow transplant more than two years ago

Care includes:

• Educating survivors about their individualized therapy and recommended follow up
• Creating a plan for preventative care and surveillance to address possible late effects of treatment
• Providing guidance on healthy lifestyle and good nutrition
• Connecting survivors with community resources
• Referring to other subspecialists when needed
• Working with survivors, their families/caregivers and their primary care doctor to meet ongoing needs

Support Services

There are many services to help support you throughout your survivorship experience. Including our Adult Oncology Group (ages 18-39), the Adolescent and Young Adult Cancer Program (for certain patients aged 15-39), our Fertility Navigation team, psychologists, social workers and more.
